ACKNOWLEDGEMENT OF RESPONSIBILITIES OF EMERGENCY EVACUATION SCHEME:
I, [the above named] on behalf of [Group/Organisation - named below] acknowledge that I have read and understood my responsibilities relating to the Emergency Evacuation Scheme for St Peters Presbyterian Church, in particular, the responsibilities of the Building Warden and Floor Warden which will be appointed at the time of the hire contract of St Peters Presbyterian Church.

OUR DUTY MANAGER WILL BRIEF YOU ON ARRIVAL, HOWEVER, YOUR RESPONSIBILITIES WILL INCLUDE:
- The appointment of Fire Wardens
- The briefing of Wardens and Occupants on the Evacuation Procedure
- Where necessary, the conducting of a Fire Drill
Floor Wardens:
On hearing the evacuation signal:
- Assist all occupants in your area to leave the building via the nearest safe exit.
- Ensure smoke control doors are closed.
- Check all areas (including toilets, storerooms, etc).
- Ensure critical appliances and systems (such as heaters and gas) are turned off (computers and lights may be left on).
- Watch for disabled people who may require assistance.
- When your area is clear, report and advise the Building Warden at the Assembly Area.
Building Warden:
On hearing the evacuation signal:
- Ensure the Fire Service has been called (111).
- Report to the Assembly Area and take charge.
- Receive status reports from floor wardens, and initiate action if no report received from any floor warden(s).
Evacuation Procedure:
1. Sound the Alarm.
2. Call the Fire Service (111).
3. Leave the premises immediately by the nearest safe exit.
4. Report to the Assembly Area.
5. Remain at the Assembly Area until the Chief Warden or the Fire Service gives the “All Clear”.
Only if conditions permit and it is safe to do so should you ever attempt to extinguish a fire.
